Targeted Case Management
Adult Mental Health Targeted Case Management (AMH-TCM) services help those in the greater Metro area diagnosed with a Serious and Persistent Mental Illness (SPMI) gain access to medical, social, educational, vocational, and other necessary services related to the person’s mental health needs.
Program Eligibility & Services
- Specialized team that provides AMH-TCM via ASL to those who are deaf, deaf/blind, and/or hard of hearing.
- Case managers meet monthly with clients to assess, plan, refer & link, and monitor and coordinate.
- Services delivered in-home, community, and or various office locations throughout the Twin Cities and St. Louis County.
Program Eligibility & Services
- Adults 18 years of age or older who live in Anoka, Dakota, Hennepin, Ramsey, St. Louis, and Washington counties in Minnesota
- Must complete an MNCHOICES assessment and be approved for waiver-funded services or Rule 185 case management through your county of residence.
- Diagnosis of a serious mental illness, substance use disorder, or co-occurring disorders
- Individuals must have a Community Alternatives for Disabled Individuals (CADI) or Brain Injury (BI) waiver.
Waiver Case Management
A service that provides people and their families access to person-centered planning, support planning, and monitoring.
Referrals
Call the Central Access Contact Center at 651-774-0011, Monday through Friday, from 7:00 a.m. to 5:00 p.m. to learn more. Waiver case management referrals must come directly from each county.
