Targeted and Waiver Case Management Services

Targeted Case Management

Mental health targeted case management (AMH-TCM) services help adults with serious mental illness (SMI) gain access to medical, social, educational, vocational, and other necessary services connected to the person’s mental health needs.

Program Eligibility & Services

  • Adults 18 years of age or older who live in Anoka, Dakota, Hennepin, and Ramsey counties in Minnesota
  • Diagnosis of a serious mental illness, substance use disorder, or co-occurring disorders
  • Services may include developing a support plan and the use of other resources and services in the community

Program Eligibility & Services

  • Adults 18 years of age or older who live in Anoka, Dakota, Hennepin, Ramsey, St. Louis, and Washington counties in Minnesota
  • Must complete an MNCHOICES assessment and be approved for waiver-funded services or Rule 185 case management through your county of residence.
  • Diagnosis of a serious mental illness, substance use disorder, or co-occurring disorders
  • Individuals must have a Community Alternatives for Disabled Individuals (CADI) or Brain Injury (BI) waiver.

Waiver Case Management

A service that provides people and their families access to person-centered planning, support planning, and monitoring.

Referrals

Call the Central Access Contact Center at 651-774-0011, Monday through Friday, from 7:00 a.m. to 5:00 p.m. to learn more. Waiver case management referrals must come directly from each county.
